Request to Cancel Housing Contract

This form is for students who have already selected a room during the 2008-2009 sign-up process and are now wishing to cancel that selection. If you want to cancel a contract for the remainder of this term, please contact your RA. If you wish to live off campus in 2008-2009 and are eligible(PDF) to do so, don't sign up for a room for 2008-2009 and your housing deposit minus any housing damage charges will be refunded to your university account.

Note: If you cancel your housing contract after May 1, 2008 your deposit is forfeited. Please review the current housing contract(PDF) to see if you qualify for a return of your deposit.

I am familiar with the Board of Regents' Mandatory Housing Policy(external) requiring that all non-exempt Freshmen and Sophomores must live on campus or commute as defined in the Housing and Dining Contract(PDF). I also understand that falsification of University records is a violation of the UW System Administrative Code Chapter 17.

Your mail during the summer months will be sent to your mailing address currently listed in eSIS(external). If this address is NOT where you want your mail sent, it is your responsibility to change this address.
